Our wide range of services help vulnerable people to thrive, through one to one health support, group activities and via our online community platforms.
In hospitals and the community we:
- help patients on hospital wards, offering companionship and support alongside NHS teams
- work with NHS Health & Social Care teams to help and support those returning home after a stay in hospital
- work with people at home to support their recovery and avoid hospital re-admissions
- help vulnerable people regain their strength and get back on their feet after an illness or operation
- provide safe transport to medical appointments and help people access the services they need
- connect people to their communities through a wide selection of social activities
- provide support those with long-term health conditions to stay physically and mentally active, through guided exercise and activity packs and online via our Virtual Village Hall.
